Our experience in health communications, change management, employee and stakeholder engagement, social marketing, research and evaluation and design and digital solutions has been built over the last two decades.
We have worked with a range of public health agencies and not-for-profit clients on everything from local community engagement plans to large scale mass-media social marketing programs.

Change Management:
Cancer Institute NSW
Realise Change is currently working with the Cancer Institute NSW to build change capability across the organisation and embed change management as part of business-as-usual activities, including:
- Developing and delivering tailored change management training and mentoring programs for staff
- Workshop facilitation and engagement with internal and external stakeholders
- Providing strategic change management advice and leadership including conducting change readiness assessments and stakeholder mapping workshops
- Developing and implementing change management strategies and leading the implementation of large-scale change programs
- Leading the co-design of bespoke tools, templates and resources to support staff managing change
Strategic reset and planning:
Multicultural Health and Hepatitis Service (Sydney Local Health District)
Realise Change is working with the Multicultural Health and Hepatitis Service (MHAHS) to reset their strategic focus and increase access to prevention, testing and treatment for people from culturally and linguistically diverse (CALD) backgrounds in line with NSW Strategies for HIV, Hepatitis B, Hepatitis C and Sexually Transmitted Infections, including:
- The delivery of an innovative and strategic 3-year roadmap with an accompanying one year work plan and an ongoing monitoring framework
- A strategically integrated program of work that responds to the changing trends in blood-borne viruses and sexually transmitted infections
- Strategies to strengthen partnerships with culturally and linguistically diverse organisations to focus efforts and achieve specific outcomes for diverse communities
- Activities aimed at increasing hepatitis C and hepatitis B testing among culturally and linguistically diverse populations across a range of settings
- Defining the unique value proposition of MHAHS and how to leverage this to lead important interventions in achieving the virtual elimination of HIV transmission in NSW by 2025
- A review the organisation’s key areas of work
Communications planning and delivery:
NSW Health
Realise Change worked closely with the Centre for Population Health within NSW Health to develop Communication Planning Toolkits (including tips and templates) that support staff through all stages of the communication planning process, including:
- Communication objectives
- Context and situation analysis
- Mapping and prioritising audiences, stakeholders, partners and influencers
- Understanding existing audience insights, conducting additional research and segmenting audiences
- Developing key messages and calls to action
- Identifying the most effective communication channels and delivery formats and how they can be resourced
- Developing communications plans
- Logic models and evaluation plans to measure the effectiveness of communications.
Stakeholder analysis and engagement:
Agency for Clinical Innovation
Recent stakeholder engagement projects include facilitating a SWOT Analysis workshop for the Agency for Clinical Innovation to identify their strengths, weaknesses, opportunities and threats both internally and externally.
Building on the SWOT Analysis, Realise Change was also employed to conduct a stakeholder mapping exercise for the Agency of Clinical Innovation. This involved working with the organisation’s leadership team to identify and map stakeholders according to their level of participation, interest, and influence.
The analysis and insight from this exercise will help inform the organisation’s External Communications Strategy and Strategic Plan and help determine how best to involve and communicate with each of these stakeholder groups.
